Taking advantage of New England Propane's value-added services is quick and easy! Simply download the forms you need below and return them to us via standard mail, fax or email*.

Instructions:

  1. Find the form you need from the selection below, and double click to open it in your browser.
  2. Right click over the form and select “Save As” to download and save it to your PC.
  3. Click and open the downloaded saved file, which brings it up in Adobe Acrobat.
  4. Fill in the form on the saved file, and resave when finished.
  5. You can then print or email the form. If a signature is required, be sure to print, sign, scan and then send us the scanned and signed copy.
